Supporting a Service User
Working in an inclusive manner, this service provides support services to young people, their families and carers, who are in transition in a way that enables young disabled people to prepare for independent living. This is provided through one to support, group support and training.
The service is tailor made based on individual's needs, wants and aspirations to ensure that a full understanding of what people want to achieve is gained and a way forward planned to ensure goals are reached and independence is developed and maintained.
